Applying for a motion to compel

In a current civil judgement, the debtor was served the "Financial Statement of Debtor" (form 13), with instructions that it must be completed and returned to the creditior within 15 days. The debtor failed to meet this deadline. I was told that a motion to compel the documents would be the next step, but how do I go about applying to the courts for this? What forms and documents do i need?
